C18 =(C11/365>5)*(C11/365-5)*9+LOOKUP(C11/365,{0,1,3,5},{0,7,14,21})*(C11/365)

Thank you so much for this, but we need the values for each of the criteria (green colored cells) and then the total of the criteria will be in C18, C35 & C52 respectively.
