Calculate DA on Multiple Conditions.

Hi all, i am having DA Table which is attached. I'm looking for a formula which will display the result in the Output table shown in Column N, O, P & Q as per the DA Table. The Column L & M will be entered manually.

Row 3 and Row 14 in the DA have same date & PO Number, but different DA% why and which should take precedence?

same with other rows
 
While Calculating DA% we should consider the Year and B_Pay in Output Table. if the B_Pay is less than 500 then it falls under 3rd Row category & it should take the 3rd row DA% and if the B_Pay is more than 500 then it falls under 14th Row Category & it should take the 14th Row DA%. However it should be within the Minimum and maximum amount limits (upon calculating DA% with B_Pay in Out-Put table) shown in Col.E & Col.F.
