Create new sheet, copy data from userform, paste it back to source workbook

Hello

I have a requirement where user enter data into an userform, click submit button and below actions are pending & need your help.

1. Create new workbook/sheet and load data from userform
2. Depends on (no.of entries) field, copy that row to those # of lines
3. Float a button called "Submit" on top of the newly created workbook/sheet and on click
4. copy entire data and load into the source workbook.

I have attached a sample file. Please help.

I have not programmed it to check date format. so you may end up with text format depending on mm/dd/yyyy or dd/mm/yyyy.

Also not programmed to check for blank entries

But have programmed it so you don't need a new sheet. It will direct paste from userform.
